PERFORMANCE CONTRACT

THIS AGREEMENT entered into by Shaw University of Raleigh, North Carolina, a corporation existing under the laws of the State of North Carolina, hereinafter called "Company" and __________ ,the Contracting Agency and Sponsor hereinafter called "SPONSOR".

UNIVERSITY SERVICES

The Sponsor hereby employs the Company to render personal services to the SPONSOR, to wit: ___________________________will deliver _________________________________ performance (s) of: _______________________.

FEES AND ARRANGEMENTS

The SPONSOR will be responsible for all expenses to be incurred above and beyond the "flat fee" performance rate: travel, lodging, food, equipment rental.  The flat fee performance rate for the above show is:

CONDITIONS

The UNIVERSITY will not be liable for any unforeseen problems or any problems not of its own cause, nor will the UNIVERSITY be liable for injuries sustained by any patron who is upon the premises during the course of the performance.

In Witness Whereof, Shaw University, and _________________ have caused their respective duly authorized officer to execute this Contract.
